A fatal accident occurred at a scenic tourist site in Sichuan, where a woman fell from height during an adventure activity after an apparent safety failure.

According to reports, the woman was participating in a high-altitude zipline-style attraction while wearing a helmet and harness. Video footage shows her suspended on a pulley system moving toward a barrier when she shouted that the rope was not properly secured.

Moments later, the safety line detached and she fell from a significant height. Witnesses nearby reacted as the incident unfolded.

Authorities confirmed the victim was seriously injured and died while being transported to hospital.

The incident took place at a waterfall scenic area in Huaying City. Officials stated the attraction had been undergoing maintenance, and the park has since been closed for safety rectification.

Authorities have launched an investigation to determine the exact cause of the equipment failure and to identify those responsible.
